This Dear Colleague letter announces the publication of two new HIV surveillance reports and an AtlasPlus update. The COVID-19 pandemic in the United States led to disruptions in HIV testing services and access to clinical services throughout 2020. This disruption resulted in a steep, single-year decline in HIV diagnoses that is mostly attributed to declines in testing caused by less frequent visits to health centers, reduced outreach services, and shifting of public health staff to COVID-19 response activities. Given these disruptions, data for 2020 should be interpreted with caution.

June 5, 2021 marks 40 years since the first official reporting, in CDC's Morbidity and Mortality Weekly Report (MMWR), of five cases describing what later became known as Acquired Immune Deficiency Syndrome (AIDS) and what is now HIV Stage III. Numbers of persons aged greater than or equal to 13 years living with diagnosed infection are reported numbers, not estimates. The annual HIV Surveillance Report provides an overview on the current epidemiology of HIV disease in the United States and dependent areas. CDC funds state and territorial health departments to collect surveillance data on persons diagnosed with HIV infection; all personal identifiers are removed from these data before being transmitted to CDC via a secure data network. Data are analyzed by CDC and then displayed by age, race, sex, transmission category, and jurisdiction (where appropriate). Prior to the 2008 reporting year, the report was referred to as the HIV/AIDS Surveillance Report.

The annual HIV Surveillance Report provides an overview on the current epidemiology of HIV disease in the United States and dependent areas. CDC funds state and territorial health departments to collect surveillance data on persons who have received a diagnosis with HIV infection; all personal identifiers are removed from these data before being transmitted to CDC via a secure data network. Data are analyzed by CDC and then displayed by age, race, sex, transmission category, and jurisdiction (where appropriate). Prior to the 2008 reporting year, the report was referred to as the HIV/AIDS Surveillance Report. The report is published annually in the fall.

During 2008-2016, the annual number of diagnoses of HIV infection increased 3% per year among MSM aged 13-29 years, decreased 4% per year among those aged 30-49 years and was stable for MSM aged ≥50 years. The number of HIV diagnoses among MSM aged 13-29 years was four times that of MSM aged ≥50 years. During 2008-2015, the number of MSM aged ≥50 years living with diagnosed HIV infection (prevalence of HIV infection) increased an average of 11% per year and at year-end 2015 was three times that of MSM aged 13-29 years.

In a survey of health care providers, 49% rated their knowledge of PrEP as poor, and 29% reported ever having prescribed PrEP.

In 2014, fewer blacks living with diagnosed HIV infection had sustained viral suppression (all viral load test results in 2014 less than 200 HIV RNA copies per mL) compared with Hispanics and whites. Among those who were in care and did not achieve sustained viral suppression, blacks had viral loads greater than 1,500 copies per mL for approximately half of the 12-month period in 2014; this circumstance can adversely affect their health outcomes and pose a risk for further transmission. Blacks aged 13-24 years had the lowest prevalence of sustained viral suppression.

Analysis of data from Population-based HIV Impact Assessment surveys conducted during 2015-2017 in seven countries in Eastern and Southern Africa found that the prevalence of HIV infection among adolescent girls and young women was 3.6%. Among those who were HIV-positive, 46.3% reported being aware of their status, and among those aware of their HIV-positive status, 85.5% reported current antiretroviral treatment (ART) use. Overall, viral load suppression among HIV-infected adolescent girls and young women, regardless of status awareness or current use of ART, was 45.0%, well below the UNAIDS target of 73%.

In 2015, National HIV Behavioral Surveillance found a 7% prevalence of HIV infection among persons who inject drugs which was lower than in 2012 (11%). Among HIV-negative respondents, 27% reported sharing syringes and 67% reported having vaginal sex without a condom in the previous 12 months; only 52% received syringes from a syringe services program and 34% received all syringes from sterile sources. HIV infection prevalence was higher among blacks (11%) than whites (6%) but more white persons who inject drugs shared syringes (white: 39%; black: 17%) and injection equipment (white: 61%; black: 41%) in the previous 12 months.

Analysis of 2015 data on CDC-funded HIV tests and HIV prevention services from 61 health departments and 123 community-based organizations indicated that among women identified as having HIV infection, 62% had received a diagnosis of HIV infection before the current test, and 87% of those women were not in HIV medical care at the time of the test. Rates for linkage to medical care within 90 days of the current test date were 61% and 58% for women with newly diagnosed and previously diagnosed HIV infection, respectively. Among women with previously diagnosed HIV infection, 57% of black women and 65% of white women were linked to HIV medical care.

On April 19, 2017, the Centers for Disease Control and Prevention (CDC) held the HIV in the South town hall meeting to elicit individual participant feedback on addressing HIV-related health disparities in Deep South states (Alabama, Florida, Georgia, Louisiana, Mississippi, North Carolina, South Carolina, Tennessee, and Texas). The meeting's town hall format allowed invited participants-who were individuals with experience and expertise in conducting HIV prevention efforts in these states-to provide l feedback on their challenges, experiences, and suggestions for addressing HIV-related health disparities in the South. The meeting notes provide an overview of key presentations and a summary of the facilitated and roundtable discussions. In 2015, 58.1% of HIV infections among Hispanics or Latinos aged ≥13 years with diagnosed HIV infection in 38 jurisdictions with complete laboratory reporting were diagnosed at an earlier stage (stage 1 or 2) and another 18.8% at an unknown stage; 75.4% were linked to care within 1 month of diagnosis. Among Hispanics or Latinos living with diagnosed HIV infection at year-end 2014, 70.2% received care, 58.3% were retained in care, and 58.2% were virally suppressed.

CDC's U.S. Medical Eligibility Criteria for Contraceptive Use (U.S. MEC) (first published in 2010 and updated in 2016) provides evidence-based guidance for the safe use of contraceptive methods among U.S. women with certain characteristics or medical conditions (1), and is adapted from global guidance from the World Health Organization (WHO) and kept up to date based on continual review of published literature (2). CDC recently evaluated the evidence and the updated WHO guidance on the risk for human immunodeficiency virus (HIV) acquisition among women using hormonal contraception. After careful review, CDC adopted the updated WHO guidance for inclusion in the U.S. MEC guidance; this guidance states that the advantages of progestin-only injectable contraceptive use (including depot medroxyprogesterone acetate [DMPA]) by women at high risk for HIV infection outweigh the theoretical or proven risks (U.S. MEC category 2).

Among black women aged 45-54 years, the human immunodeficiency virus (HIV) disease death rate decreased 60% from 28.4 per 100,000 in 2006 to 11.5 in 2015. Among black women aged 55-64 years, the rate increased 42% from 10.0 in 2000 to 14.2 in 2008, before declining to 10.3 in 2015. Among white women aged 45-54 years, the rate decreased 53% from 1.9 in 2005 to 0.9 in 2015. Among white women aged 55-64 years, the rate did not change, remaining at about 0.8. Throughout the period, HIV disease death rates among black women were higher compared with rates among white women for both age groups.

Today, the Centers for Disease Control and Prevention (CDC) released estimates that show if current HIV diagnoses rates persist, approximately 1 in 2 black gay and bisexual men and 1 in 4 Latino gay and bisexual men in the United States will be diagnosed with HIV during their lifetime.

Cluster of HIV Infections Attributed to Unsafe Injection Practices - Cambodia, December 1, 2014-February 28, 2015

The largest cluster of new HIV infections ever attributed to unsafe injections among a general population was reported in a rural area of Cambodia; 2.7% of residents were infected. The outbreak was detected after increased demand for HIV testing by residents who perceived themselves to be at risk after exposure to an unlicensed provider of injections and intravenous infusions.

Gay, bisexual, and other men who have sex with men (MSM) are affected disproportionately by human immunodeficiency virus (HIV) in the United States. Although approximately 3% of the adolescent and adult U.S. male population is estimated to have engaged in same-sex behavior in the past year (1), in 2011, MSM accounted for 65% of the estimated 49,273 new HIV infections and 82% of the estimated 38,825 HIV diagnoses among all males aged ≥13 years (2). Sexual risk behavior accounts for most HIV infections among MSM, and anal intercourse without a condom is the primary route for transmitting HIV infection to an uninfected person.

Hispanics/Latinos in the United States are affected disproportionately by human immunodeficiency virus (HIV) infection, acquired immunodeficiency syndrome (AIDS), and other sexually transmitted diseases (STDs); however, few effective evidence-based prevention interventions for this population exist. This report describes the Hombres Manteniendo Bienestar y Relaciones Saludables (Men Maintaining Wellbeing and Healthy Relationships) (HoMBReS) intervention, which was developed by a community-based, participatory research partnership in North Carolina and initially implemented during 2005-2009.

MMWR: Occupational HIV Transmission Among Male Adult Film Performers - Multiple States, 2014

This is the first well-documented work-related HIV transmission among male adult film performers. A performer was infected by a non-work-related partner who was not aware of his HIV infection. The performer, having tested negative by nucleic acid amplification test within the preceding 14 days, and unaware of his very recent HIV infection, infected another performer and a non-work-related partner. Viruses in all four HIV infections were highly genetically related, indicating a transmission cluster.

The findings in this report do not provide evidence that HIV-related risk behaviors alone drive the higher numbers of HIV diagnoses among young black MSM compared with young Hispanic and white MSM. In fact, young black male students who had sexual contact with males in this report often had a lower prevalence of HIV-related risk behaviors.
